Weekly Larry Ventis Pro Invitational Participation Request

Fill out and submit the following form to request to participate in the Weekly Pro Invitational.
Participation requests are accepted from 1:30 pm Tuesday until 5:00 pm Sunday for the following week’s event.
Requests are first come, first served.
Participation is limited due to court and pro availability along with the number of any previously received requests, so you may or may not be able to participate.
You will receive a participation confirmation email on Monday the day before the event if you were added to the roster.
